The Children & Young People’s Health Partnership (CYPHP) Health Check is for parents/carers of children:
-With Asthma, Constipation, Eczema or Epilepsy; and
-Registered with a GP in Southwark or Lambeth; and
0–15 years old
It takes about 5 minutes to complete.
